How to Request a New Phone Number
Go to ITS Service Desk
Provide the following information
Name, NetID, and Email Address of person to be assigned to the phone
Will the person only need a Softphone (Jabber)
If they also need a Desk Phone, provide the MAC Address of the phone
Do they need to answer additional lines (up to 4), if so then provide the extensions of the additional lines
